Responsibilities

Provide home‑based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury.

Work as part of a home‑health team (RN, LPN, CNA, HHA, OT, SLP) focused on the patient’s plan of care and goals.

Observe movements, conduct assessments, document progress and modify care plan as needed.

Assist patients with exercises and hands‑on therapy to manage pain and increase mobility.

Educate patients on their plan of care, established goals, exercises and self‑care.

Coach family members on the plan of care, therapy goals and how to safely assist with exercises.

Qualifications

Graduate of an accredited Physical Therapist program and holds an active PT license in Texas.

Minimum of 1 year of physical therapy experience, ideally in home healthcare.

CPR certification.

Knowledge of state and federal home‑health regulations.

Good clinical judgement, strong interpersonal skills, dexterity and compassion.
