The LPN Intake Nurse - Home Health contributes to our Mission of Caring by receiving and reviewing incoming referrals for home health services, facilitates timely and accurate onboarding, and ensures efficient coordination among field-based clinical staff. Qualifications for a LPN Intake Nurse - Home Health:
Unencumbered, active LPN license issued by the RI DOH.
At least 3 years of experience in a healthcare or home health care setting, with a focus on patient intake or coordination is preferred.
Ability to work independently with minimal supervision.
Knowledge of healthcare regulations and insurance guidelines.
Strong knowledge of medical terminology, disease processes, and treatment options.
Proficient in accessing and interpreting electronic health records.
