Opening in November at our newly renovated facility at 4740 A Street, the Short-Term Residential program represents an exciting new chapter for Houses of Hope. We are building a team of professionals who want to create a safe, welcoming, trauma-informed environment where individuals experiencing substance use and co-occurring mental health needs can take meaningful steps toward recovery and greater independence.
Joining the program at its beginning offers a unique opportunity to help establish the culture of a new team and contribute to how services are delivered from day one. Houses of Hope is seeking a social worker to join our team. This position supports individuals in the Short-Term Residential program by coordinating services, advocating for their needs, and facilitating treatment and discharge planning in collaboration with the treatment team.
Duties and Responsibilities Foster and maintain a safe, welcoming, trauma-informed environment for all individuals served.
Maintain confidentiality in accordance with federal confidentiality laws, HIPAA regulations, agency policies, and professional therapeutic boundaries.
Work closely with the treatment team to facilitate treatment goals, service coordination, and discharge planning.
Provide support, logistical assistance, and advocacy for individuals in service and, when needed, their families.
Coordinate access to medical, social, educational, housing, transportation, and other appropriate support services, including linkage to more or less intensive community services.
meet with persons served to discuss and support progress toward identified goals.
Maintain clinical records and complete required tasks and documentation accurately and within agency-established timelines.
Conduct post-discharge follow-up with individuals to assess continuity of care, confirm receipt of needed supports, and identify potential barriers.
Establish and maintain effective working relationships with community agencies, organizations, providers, and individuals.
Maintain current knowledge and a working database of community resources and service connections.
Maintain positive staff relationships, work actively as a team member, and communicate regularly with counseling, clinical, and administrative staff.

CPR/First Aid Certified (will provide training)
Bachelor’s Degree in social work
Commitment to trauma-informed, person-centered, and culturally responsive care
Strong verbal and written communication skills, including skills required for documentation in clinical records
Ability to establish rapport and effective working relationships with individuals, families, colleagues, community providers, and other stakeholders
