Stroke Nurse Coordinator

Under the direction of the Medical Director and Stroke Program Administrator and in collaboration with team members, the Stroke Nurse Coordinator is responsible for all aspects of care of the stroke patient across the continuum. As a key member of the multidisciplinary team, the Stroke Coordinator continually analyzes and monitors practices and program guidelines consistent with the criteria outlined in national stroke certification manuals (i.e. The Stroke Coordinator is active in clinical, educational, quality improvement, research, and stroke registry components of the Stroke Program.
Assures the delivery of comprehensive stroke care across the continuum (pre hospital, emergency care, acute treatment, hospitalization, and post-discharge), through coordination of patient care consistent with stroke program clinical practice guidelines.
Rounds with the medical team and collaborates with the nursing staff to ensure evidence-based care and compliance with performance measures.
Attends Stroke Alerts, as able, to support the team and evaluate adherence to care pathways.
Performs patient follow-up calls, collecting clinical information and patient experience data.
Monitors, communicates effectively, and mentors other team members regarding patient needs, plan of care and response to the treatment plan.
Along with the Stroke Program Medical Director and Stroke Program Administrator, ensures maintenance of the Advanced Disease-Specific Care Certification Standards (i.e. Provides follow-up and support for patients with stroke in non-neuro areas of the hospital or units outside of the neurosciences.
Serves as an education resource in stroke care and creates a positive learning environment for patients, physicians, nurses, case managers, ancillary and support staff.
Provides regular education on stroke to EMS partners, stroke survivors, and our surrounding community.
Mentors team member to maintain appropriate documentation of patient care and progress within the established plan to meet professional and institutional guidelines.
Quality Improvement:
Works with Stroke Program data abstractors to ensure accuracy and integrity of stroke registry information.
Identifies and reports gaps in performance indicators; implements plan of action to improve compliance with indicators, including quality improvement initiatives.
Prepares and sends care feedback reports to teams who treat patient with IV tPA, thrombectomy, and more.
Identifies the need for, arranges, and conducts team meetings to facilitate team communication and coordination of care.
Participates in stroke-related research, as appropriate, through identification of subjects, data collection and chart reviews, and other duties as directed by the Principal Investigator.
Collaborates with team members, identifies trends and best practices in healthcare research and education as they relate to the care of the stroke patient.

~ Bachelor's Degree in Nursing; Minimum of three years of clinical nursing experience
At least two years in the area of neuro/stroke
Two or more years' experience with project management and/or quality improvement.
Critical care course completed successfully within six months.
Must complete the acute stroke unit specific competencies.
Track record of working successfully with physicians and healthcare teams in collaborative practice model.
Demonstrated expert clinical knowledge of care management of the stroke patient and family.
Understanding and application of concepts of strategic planning, quality improvement principles, and quantitative analysis.
Ability to take necessary actions to develop and maintain personal job competencies/expertise and to improve performance on an ongoing basis; keep current regarding medical and technological trends in stroke care.
Demonstrated excellent coaching and mentoring skills.
Current RN licensure in the state of Michigan.
Stroke-certified RN (SCRN) or neuro-certified RN (CNRN) within two years from hire.
