Description

Conduct assessments for patients in inpatient and outpatient environments and develop tailored treatment plans
Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function
Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
Provide education to patients and families regarding therapeutic objectives, discharge planning, and recommended home exercises
Maintain comprehensive documentation of evaluations, treatments, and progress in compliance with facility policies and regulatory standards
Engage in care team rounds and interdisciplinary meetings to ensure cohesive care delivery
Education
~ Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Additional certifications (e.g., neurological, geriatric) are beneficial but not mandatory
Skills
Previous experience is highly desirable
