LVN, Home Health Care - Full-Time, Part-Time or PRN

Job Summary
Licensed Practical Nurse Seeking a Licensed Practical Nurse/LPN who will be responsible for providing direct patient care . Will be responsible for working collaboratively with the members of the healthcare team to help meet positive patient care outcomes.

Responsibilities and Duties
Responsibilities of Licensed Practical Nurse/LPN include but not limited to the following:

Implement plan of care initiated by the clinician

Provides accurate and timely documentation

Assess and provides patient and family caregiver education and information pertinent to diagnosis and plan of care

Participates in coordination of home health services to the clinician and/or clinical supervisor

Qualifications and skills:

One year med surg experience

One year Home Health experience required.

Louisiana State Board of LPN license required
