Physical Therapist (PT) (Full Time, Day Shift)

Description: The Physical Therapist provides evaluation and treatment of patients referred for physical therapy services. The therapist communicates and collaborates with other caregivers to provide quality care and maintains accurate patient records.
What You Will Do Perform physical therapy evaluation and establish and implement treatment plans for patients referred by providers and assigned by the manager, clinical coordinator or designee.
Document accurately PT evaluation results, treatment interventions, patient progress toward goals, and discharge records.
Maintain proper billing for services provided.
Delegate appropriate treatment procedures to the Physical Therapist Assistant/PTA, reviewing and co-signing their documentation.
Communicate with appropriate health care providers on matters concerning patient treatment, continuity of care, and discharge planning.
Graduate of an accredited physical therapy program (Doctorate degree preferred).
Current license to practice Physical Therapy in the state of Ohio; current Basic Life Support (BLS) for Health Care Provider certification required.
