Home Care Full-time Occupational Therapist

Role: Occupational Therapist (OT)

Schedule: Full-time Monday-Friday + occasional weekends (as needed)

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model. Our ultimate mission is to dramatically improve patient access to home health care.

The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs.

We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. If you have strong clinical judgment and you like owning outcomes, this role gives you autonomy, meaningful patient impact, and a support team built to remove administrative friction.

Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.

Recommend and train on adaptive equipment and home modifications as needed.

Coordinate with nursing and the broader care team when condition or needs change.

Document promptly and thoroughly - defensible, efficient, and consistent.

Strong clinical judgment and comfort working independently in the field

Comfort escalating changes in condition to RN/MD and documenting rationale clearly

Current driver’s license and comfort with daily travel across assigned territory

Able to carry/transport basic therapy equipment as needed

Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Prior home health experience

Homecare Homebase (HCHB) experience (or similar home health EMR)

Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
