Job Description The Avenue at Westshore Care and Rehabilitation Center is committed to providing an excellent level of physical, medical, emotional, and spiritual support in an environment of comfort and respect. Our facility provides some of the area’s highest levels of rehabilitation, nursing staff and medical support to individuals needing post-hospital care and services. Monitor patient’s condition and assess their needs to provide the best possible care and advice

Observe and interpret patient’s symptoms and communicate them to physicians

Perform skilled charting via the point click care charting system

Adjust and administer patient’s medication and provide treatments according to physician’s orders

Maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)

Provide instant medical care in emergencies

Supervise nursing assistants

Foster a supportive and compassionate environment to care for residents and their families
