Key Responsibilities for Physical Therapist (PT):

Provide skilled physical therapy interventions to improve strength, balance, mobility, and functional independence.
Document all evaluations, treatments, and progress notes accurately and in compliance with agency and regulatory standards .
Collaborate with physicians, nurses, and other care team members to ensure seamless continuity of care.
Provide professional guidance and support to patients and their families to encourage active participation in care.
Maintain equipment, supplies, and records necessary for home therapy sessions.
Qualifications for Physical Therapist (PT):

Licensed Physical Therapist (PT) in Massachusetts.
Previous experience in home health or outpatient physical therapy preferred.
Ability to work independently and manage a flexible schedule.
