Position Overview

A growing home health organization is seeking a Registered Nurse to provide skilled nursing services to patients in their homes. This role offers the opportunity to work one-on-one with patients, develop meaningful clinical relationships, and deliver personalized care in a home-based setting.

Schedule:

Full-time, 40 hours per week

Day shift: 8:30 AM - 5:00 PM

Every fourth weekend

Two holidays out of six annually

No on-call responsibilities

Key Responsibilities

Conduct comprehensive patient assessments and develop individualized plans of care.

Deliver skilled nursing services in accordance with physician orders, clinical standards, and regulatory requirements.

Monitor patient progress and adjust care plans as needed based on changes in condition.

Perform medication reconciliation and collaborate with providers regarding treatment plans.

Ensure timely and accurate clinical documentation within established organizational guidelines.

Provide patient and caregiver education to support treatment goals and promote positive outcomes.

Coordinate care with physicians, therapists, home health aides, and other interdisciplinary team members.

Manage visit schedules and maintain compliance with payer authorization requirements.

Communicate patient status updates and care plan changes to appropriate healthcare team members.

Supervise home health aide staff and complete required supervisory visits.

Maintain current knowledge of nursing standards, best practices, and regulatory requirements.

Meet productivity expectations and complete assignments independently and accurately.

Perform additional duties as assigned.

Required Qualifications

Associate Degree in Nursing or Nursing Diploma.

Minimum of six (6) months of Registered Nurse experience OR at least one (1) year of Licensed Practical Nurse experience.

Experience using computers and electronic documentation systems.

Active New Jersey Registered Nurse (RN) license.

American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers certification.

Preferred Qualifications

Bachelor of Science in Nursing (BSN).

Home health or hospice nursing experience.

IV Certification.
