Licensed Social Worker (LSW) for our Skilled Nursing Facility (SNF)
Compensation: $60,000 - $80,000 Annual Salary

We are looking for a Licensed Social Worker to support residents and families at our skilled nursing facility in Ellicott City, MD. This is a full-time, on-site role handling psychosocial assessments, care planning, discharge planning, and resident and family support as part of the interdisciplinary team.
Qualifications: Active Maryland social work license (LMSW, LGSW, or LCSW-C)
Experience in skilled nursing, long-term care, or a healthcare setting preferred
Knowledge of psychosocial assessment, care planning, and discharge planning
Strong communication and documentation skills
Responsibilities: Complete psychosocial assessments and MDS social work sections
Develop and update care plans with the interdisciplinary team
Coordinate discharge planning and community resources
Support residents and families through admission, stay, and transitions
Maintain accurate documentation and regulatory compliance
