Registered Nurse Radiology/ Pain Management
Seeking a compassionate and experienced Registered Nurse to join our dynamic Discharge team. In this role, you will play a vital role in ensuring a smooth and safe transition for patients as they prepare for discharge. You will collaborate closely with patients, families, physicians, and other members of the healthcare team to provide high-quality, patient-centered care.
Key Responsibilities:

Assess patient needs and develop individualized discharge plans.
Provide education to patients and families regarding medications, wound care, diet, and other discharge instructions.
Coordinate discharge transportation and home health services as needed.
Address patient concerns and provide emotional support during the discharge process.
Monitor patient vital signs and assess for any potential complications.
Maintain accurate and timely documentation in the electronic medical record.
Collaborate effectively with the interdisciplinary healthcare team.
Adhere to all hospital policies, procedures, and safety protocols.
Participate in quality improvement initiatives and professional development activities.
Qualifications:

Graduate from an accredited school of nursing with a current New York State RN license.
Bachelor of Science in Nursing (BSN) required.
One year of acute care nursing experience preferred.
BLS certification required.
ACLS certification required (within 12 months of hire for new hires).
Excellent communication, interpersonal, and patient education skills.
Strong organizational and time-management skills.
Ability to work independently and as part of a team.
