As a Home Health RN, you will:

Complete skilled nursing visits in patients' homes

Perform comprehensive nursing assessments

Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned

Provide wound care and medication management

Communicate changes and concerns to physicians and the care team

Coordinate with therapists and other members of the interdisciplinary team

Help identify concerns early to reduce unnecessary hospitalizations

Active Virginia Registered Nurse License Required

Graduate of an accredited nursing program

Ability to work independently in patients' homes

Home Health and OASIS Experience Preferred, but Not Required
