Deliver comprehensive care to individuals and families throughout their lives, from children to seniors.

MD or DO degree from an accredited medical institution.
Completion of a Family Medicine residency program accredited by ACGME.
Board Certification in Family Medicine.
Active (or eligible for) medical license in Kentucky.
