Pharmacist - Clinic Pharmacy (Full-Time)

Ambulatory PharmacistWe're unique. We're different than most primary care providers. The Ambulatory Pharmacist is a patient-centered clinical professional dedicated to optimizing medication therapy within primary care clinics. Operating under a collaborative practice agreement, this role focuses on reducing polypharmacy, enhancing medication adherence, and managing chronic diseases to improve patient outcomes and elevate quality performance metrics such as CMS Star Ratings and HEDIS scores. The ambulatory pharmacist partners closely with patients, physicians, advanced practice providers, nurses, and care teams to deliver evidence-based, individualized care that promotes safety, efficacy, and cost-effectiveness.Collaborates with healthcare providers to adjust medication therapies, interpret lab results, and support clinical decision-making within a protocol-driven framework.Chronic Disease Management: Proactively manages conditions such as diabetes, hypertension, anticoagulation, and asthma to improve control and prevent complications.Lab Testing & Monitoring: Orders, interprets, and utilizes laboratory results to monitor medication, safety and effectiveness.Administrative & Support Activities: Assists with physician consultations, provides accurate drug information, and supports the department's quality improvement initiatives.Knowledge of and experience with state and federal insurance programsFamiliarity with appropriate local, state, and federal pharmacy-related regulationsAbility to work effectively both independently and as part of a multidisciplinary teamExcellent organizational, communication, and clinical decision-making skillsStrong attention to detail and ability to manage multiple prioritiesWillingness to travel locally up to 50% of the time; flexible to work evening, weekends and/or holidays as neededFluency in spoken and written EnglishThis job requires use and exercise of independent judgmentEducation and Experience Criteria:Doctorate of Pharmacy (PharmD) requiredValid pharmacy license in good standing (Doctor of Pharmacy/PharmD) requiredPharmacy residency or ambulatory care certification (e.g. BCACP) preferredMinimum two (2) years of relevant clinical experience in primary care, ambulatory care, managed care, or related settingCertification in immunization and medication therapy management is a plusResidency training or ambulatory care certification is highly preferredExperience in managed care, HMO, PBM, or MSO settings is a plusBasic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employmentPay Range: $118,062 - $168,658 SalaryThe posted pay range represents the base hourly rate or base annual full-time salary for this position. This position may also be eligible for bonuses or commissions.
