Responsibilities
Responsible for the evaluation, treatment, progression, and discharge planning of patients throughout the hospital.

Responsible for the overall direction, utilization, supervision, instruction, and evaluation of professional and supportive staff and students in the physical therapy department/facility.

Perform patient assessment prior to initiation or renewal of therapy, including obtaining a medical history, reason for therapy, prior level of function, complications, precautions, and critical behavior/mentation.

Perform a functional assessment for each patient prior to treatment.

Write goals in functional or behavioral terms which include time frames.

Establish treatment plans based on the results of the assessment.

Observe and document patient response to treatment.

Begin patient/family education at time of assessment.

Document clearly on patient record awareness of age‑specific needs and their impact on patient assessment.

Include the patient in decision‑making process regarding treatment program.

Maintain patient treatment schedule consistent with the needs of the department/facility.

Demonstrate knowledge and understanding of age‑specific needs of the patient and design the treatment plan based on human growth development.

Explain every aspect of the written development plan of care before initiating treatment.

Contact physician when there is a question concerning the prescribed patient orders.

Adapt conventional physical therapy techniques to meet the needs of patients unable to comprehend verbal commands or voluntarily carry out a regime of therapeutic exercises.

Establish and maintain a positive line of communication with mutual respect for and trust with each patient.

Document all treatments to include patient response.

Prepare and submit written evaluation reports.

Prepare and submit written discharge summary reports when necessary.

Accurately write progress notes in patient chart.

Accurately submit procedure charges on a daily basis.

Requirements

Graduate of an approved school of Physical Therapy.

Bachelor of Science Degree in Physical Therapy or Master’s degree or higher in Physical Therapy. Doctor of Physical Therapy (DPT) preferred.

NYS PT Licensure and registration.

Previous hospital experience required.

Ability to communicate clearly and effectively with staff members, management peers, and hospital administration.

Must be able to lift and position patients.

Knowledge of all equipment used in the rehabilitation of physically impaired patients.

Prior EPIC experience.

BLS through American Heart Association.

Salary and Benefits
USD $44.41 - $55.55 per hour. The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
