Lpn- Health Care

Adecco Healthcare & Life Sciences is assisting in the placement of a Licensed Practical Nurse (LPN/LVN), Home Health opportunity in the Dickson City, PA area with a nationally recognized healthcare organization known for delivering compassionate, patient-centered home health and hospice services across the United States.
Full-Time | 32 Hours Weekly
$Provide skilled nursing visits in accordance with physician orders and individualized treatment plans

• Monitor patient conditions and report significant changes to the clinical team and physicians
• Maintain accurate and timely clinical documentation and medical records
• Educate patients and caregivers regarding treatment plans, medications, and ongoing care needs
• Coordinate care with physicians, clinicians, and interdisciplinary team members
• Support continuity of care through effective communication and follow-up
• Travel throughout assigned coverage areas to deliver patient care services
• Participate in care planning and team collaboration activities
• Ensure compliance with organizational policies and regulatory standards

Current Licensed Practical Nurse (LPN/LVN) license in the state of Pennsylvania

• Graduate of an accredited practical or vocational nursing program
• Minimum one year of clinical nursing experience required
• Minimum one year of home health experience preferred
• Ability to work independently in a field-based environment
