Advanced Practice Provider (NP/PA-C) - Heart Failure Service
Advocate Lutheran General Hospital | Park Ridge, IL Advocate Health is seeking a dedicated and collaborative Nurse Practitioner (NP) or Physician Assistant (PA-C) to join our growing Heart Failure team at Advocate Lutheran General Hospital in Park Ridge, Illinois. This role is structured as 50% outpatient and 50% inpatient, providing continuity of care for patients across the heart failure spectrum.

Outpatient Heart Failure Clinic (50%)

Manage patients recently discharged following a heart failure hospitalization to ensure smooth transitions of care and reduce readmissions.

Collaborate with cardiologists, heart failure physicians, nurses, pharmacists, and other interdisciplinary team members to optimize patient outcomes.

Assume primary APP management responsibilities for hospitalized heart failure patients after initial consultation.

Coordinate daily inpatient care, including treatment planning, medication titration, diagnostic interpretation, discharge planning, and care transitions.

Serve as a key liaison among patients, families, consulting teams, and outpatient providers to ensure seamless continuity of care.

Four 10-hour shifts per week

One Saturday hospital rounding shift every 8 weeks

This role offers an exceptional opportunity for an APP who enjoys both acute and ambulatory care and is passionate about improving outcomes for patients living with heart failure.

This includes ordering, performing, and interpreting appropriate diagnostic studies and prescription of pharmacologic and non-pharmacologic interventions and therapies.

Manages conditions based on clinical indication, evidence-based care, cost effectiveness, and assessment of risks/benefits and alternatives.

Provides health promotion, disease prevention and disease management counseling and education of patients and families.

Seeks physician or other healthcare team member consultation or referrals as appropriate. Escalates need for more emergent or specialized care when necessary.

Maintains accurate, complete, concise, and timely documentation in the electronic medical record. Facilitates consistent, coordinated care and clear communication among all members of the healthcare team and/or health or community agencies.

Performs office or hospital procedures in accordance with specialty practice, competency and granted privileges (where applicable).

Participates in quality, safety, and peer review initiatives/performance activities, organizational and/or departmental meetings and committees, peer review, and workgroups as necessary.

Participates in education and/or onboarding of new team members, students, and other health care professionals.

Maintains standards of productivity, access, face-to-face time, and quality metrics to ensure optimal, safe and timely patient care delivery.

Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. License/Registration/Certification Required
Active RN, and APRN/APRN-FPA/APNP or other APRN license(s) in state(s) of practice, AND

Active national board certification in area of clinical practice and populations served, AND

Active DEA registration prior to hire, AND

Education Required: Master’s Degree in Nursing

Experience Required: No experience required

Demonstrated ability to work independently and as an effective member of a health care team.

Demonstrated ability to adapt to evolving technology and proficiency with the electronic medical record.

Must be able to stand, walk, bend, stoop, and twist continuously throughout the workday.

Lift up to 50 lbs from floor to waist.

Operate all equipment necessary to perform the job.
