Job Description:
Seeking a Social Worker Case Manager to provide specialized care and case management for acute care hospital patients, including those in medical surgical, ICU, mother baby, labor and delivery, and ER weekend coverage. This role involves collaboration within a Level 3 Trauma Center.
Responsibilities:

Conduct EPIC experience, drug and alcohol screens (SBIRT), mental health evaluations (SAFE-T), and capacity evaluations
Develop behavior plans, conduct BH referrals, and assist with Tele Psych evaluations
Perform discharge planning, manage end of life conversations, and handle single bed certifications/ITA process
Manage difficult placements and coordinate with home health, hospice, SNFs, Medicaid/Medicare, and insurance authorizations
Requirements:

Minimum 2 years acute care hospital experience or 1 year acute care plus 1 year in hospice, home health or work with the elderly
Required Certifications: MSW, LICSW or LSWAIC (only if they do not need supervision by MHS)
Proficient in EPIC EMR system
Familiarity with medical surgical, ICU, mother baby, labor and delivery, ER patient types
