Overview
To provide practical nursing services under the supervision of a Registered Nurse.
Responsibilities
Accept assignments of clients from the R.N. Case Manager.
Provide nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans.
Perform treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
Observe signs and symptoms and report them to the R.N. Case Manager.
Note changes in the physical or emotional status of clients.
Promote client and caregiver independence in self-care.
Utilize electronic documentation of client care; comply with Agency policies regarding the timeliness of submission and synchronization of data.
Follow Agency policies regarding universal precautions and safety guidelines.
Fulfill other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.
Qualifications
Holds a current Licensed Practical Nurse license in Virginia.
Has a minimum of one year of nursing experience.
Has current CPR certification.
Excellent communication, organizational, and interpersonal skills.
Able to function with minimal supervision.
Able to manage multiple projects and changing priorities.
Able to employ critical thinking skills.
