Job Summary:

The Registered Nurse will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness in a home health and hospice environment. Key functions include patient assessments, medication administration, treatment planning, and collaborating with an interdisciplinary team. The role requires traveling to patient locations to deliver compassionate, high-quality care while maintaining strong communication with patients, providers, and team members.

Location:Spokane Valley, Washington, United States

Responsibilities:

• Provide skilled nursing care, case management, and hospice care to patients.
• Conduct comprehensive patient assessments, administer medications, and develop treatment plans.
• Collaborate effectively with the interdisciplinary team to ensure optimal patient outcomes.
• Travel to patient homes or facilities, visiting up to 6 patients per day to meet a standard of 30 productivity points.
• Document patient care and maintain accurate records using provided electronic medical record software on a provided laptop.
• Communicate effectively with patients, healthcare providers, and team members.

Required Skills & Certifications:

• Active Registered Nurse license in the state of working (pending licenses are accepted).
• Minimum of 1 year of skilled hospice experience (first-time travelers are not accepted).
• Active BLS certification.
• Experience utilizing home health electronic medical record software.
• Valid driver's license, active car insurance, and a personal automobile for field work.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Bachelor's degree in Nursing or a related field.

Preferred Skills & Certifications:

- Two or more years of hospital, home health, or hospice care experience.

Special Considerations:

• Field-based hospice role requiring personal transportation; occasional maximum driving radius of 50 or more miles.
• Patient visits are grouped to minimize drive time; mileage reimbursement is provided at the standard IRS rate.
• No on-call or floating requirements.
• Facility may call off up to one shift per pay period (every two weeks).
• Time-off restrictions apply and must be confirmed with the hiring manager.
• Required attire is any basic color scrubs.
• Local candidates within a 50 to 75-mile radius are accepted.

Scheduling:

• 8-hour day shifts.
• Immediate start date available.
• No weekend shifts required.
• Block scheduling is not accommodated.,
