As part of Saint Anthony Hospital's commitment to providing the highest quality health care, we will be building a new state-of-the-art hospital to serve as an anchor to the Focal Point Community Campus. Plans, organizes, and implements social work services for patients ranging in age from neonate to geriatrics, with particular emphasis on supporting postpartum mothers, newborns, and pediatric patients . Provides psychosocial assessments, crisis intervention, and discharge planning while promoting family-centered care and safe transitions across the continuum of care
Conducts psychosocial assessments of patients and family members to obtain information about the home environment, family relationships, perception of the patient's illness and coping resources in order to provide emotional support and discharge planning assistance. Documents same in the patient's medical record. Along with this, he/she gathers pertinent information from medical records, hospital personnel and significant others to formulate a comprehensive discharge and post-hospital plan of care. Collaborates with patients/significant others and/or physicians on discharge planning, such as nursing home placement, acute rehabilitation placement, home health, transportation, etc. Assesses patients with substance abuse/mental health problems.

# Collaborates with all adoption cases acts as a liaison between hospital and agency as needed. Performs both essential and marginal job functions in a safe manner as identified by Saint Anthony Hospital and the respective department while adhering to the Hospital's confidentiality norms and standards. Actively and consistently demonstrates the core values of Saint Anthony Hospital in all interactions with others.

• Assumes responsibility for knowing and maintaining established hospital and departmental objectives, policies/procedures including general sanitation, safety, environmental and infection control standards.
• Participates in performance activities including problem identification, data collection, solution selection, implementation and evaluation. Also assists in the care and maintenance of departmental equipment and supplies as appropriate.
