Deliver ongoing healthcare services to individuals and families throughout their lives, encompassing all ages from children to seniors.
Address chronic diseases, provide preventive care, manage acute health issues, and support mental health with a focus on patient-centered approaches.
Collaborate with healthcare coordinators, mental health practitioners, and specialists to offer integrated care.
Engage in initiatives aimed at improving patient experiences and enhancing healthcare quality.
Utilize electronic medical records (EMR) to keep precise and current patient information.
Provide care through in-person visits and telehealth options to best serve patients' preferences.
MD or DO degree from an accredited medical institution.
Completion of an accredited Family Medicine residency program.
Board Certification in Family Medicine.
Eligible for (or possess) a valid Massachusetts medical license.
Strong communication, teamwork, and relationship-building skills.
Proficient with EMR systems and telemedicine technologies.
