About This Role The Hospice Nurse Weekend (RN) provides skilled nursing care to hospice clients in their homes or community health settings, ensuring high-quality, cost-effective care. This role is ideal for compassionate nurses who want to make a real difference in the lives of patients and their families.
Department/Unit: 95005-Hartford Hospice Gen
Schedule: Part Time, Day shift
What You'll Do Provide skilled nursing care to hospice clients in home or community settings.
Conduct complete nursing assessments based on referral data and clinical observations.
Develop care plans in collaboration with clients and interdisciplinary teams.
Modify care plans as conditions change.
Direct LPNs and Home Care Aides in executing the care plan.
Participate in client care conferences and staff meetings.
Requirements Education Associate’s degree in nursing required
Experience One year nursing experience required
Home care experience preferred
