Licensed Practical Nurse - Weekends - Home Care

Are you a compassionate and skilled nurse with a passion for senior care? Join our team as a Wellness Nurse and help us deliver exceptional care to our residents. In this role, you’ll administer medications, oversee resident care, and support a team dedicated to dignity, safety, and person-centered service.
Location: Heartis Mid Cities, an Independent and Assisted Living with Memory Care senior community
Saturday & Sunday | 6:00a.m. - Administer medications safely and accurately per physician orders and regulatory standards
Oversee medication aides and conduct monthly medication reconciliation and audits
Coordinate daily staff assignments to ensure consistent resident care
Communicate effectively with physicians, families, and care teams regarding resident needs
Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support
Respond to medical and behavioral emergencies with sound clinical judgment
Document incidents and communicate promptly with leadership
Monitor care team documentation for accuracy and completeness
Support orientation and ongoing training of care staff
Maintain professional appearance and hygiene
Perform other duties assigned to support resident care and operational excellence
Qualifications: Current and valid LVN/LPN license in the state of employment
Minimum 6 months to 1 year experience in Assisted Living or Long-Term Care
Proficient in Electronic Health Records (EHR) and basic computer skills
