What You'll Do

Provide skilled, one‑on‑one nursing care in the home

Administer and manage medications

Monitor and document patient conditions

Perform treatments as ordered by the physician

Educate and support family members

Collaborate closely with the Phoenix clinical team

What You Need

Active Illinois RN or LPN

Strong assessment and communication skills

Compassionate, dependable, and self‑motivated

Valid driver's license and reliable transportation
