Schedule:PRN DAY Shift, 7a-7p. Must Be able to work a minimum of 2 shifts per pay period.
Are you a Licensed Practical Nurse looking for a new opportunity? Haven Hospice is seeking a passionate, dedicated Hospice LPN to join our team in Lake City, FL . Our hospice LPNs provide expert, patient-centered care at a critical time in people's lives. Provide 1:1 care to make a lasting impact on patients and families

~ As a Hospice LPN You will:

• Give hands-on care to patients nearing the end of life, following state nursing rules.
• Follow the care plan after assessing the patient's needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).

Take part in training and quality improvement activities to grow professionally and improve care.

*Graduate of an accredited practical nurse or vocational nursing program.

• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the State.
