Registered Nurse – Home HealthSchedule: 8 hoursAssignment Details:Contract Length: 14 weeksGuaranteed Hours: 40 per weekRequirements:Current state license as a Registered NurseHomecare Homebase Software ExperienceOASIS Experience1-2 years experience as a Registered Nurse in a clinical care setting2+ years Home health experienceProof of current CPR certificationCurrent driver's licenseMust have reliable transportation and insuranceRoles & Responsibilities:Assess patients in home health and/or hospice settings to identify physical, psychosocial, and environmental needs.Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.Educate patients and families regarding care, medications, and home safety, using age-appropriate teaching methods.Supervise and provide clinical direction to home health aides and other support staff.Manage pain through assessment and utilization of effective techniques, educating patients about pain management.Facilitate communication among patients, families, and the interdisciplinary team to coordinate care effectively.Compliance / Notes:Must meet mandatory continuing education requirements.Maintains confidentiality as required.Client Details:City: Brooklyn ParkState: MNTLC Nursing Associates, Inc. is an equal-opportunity employer and fully complies with all applicable federal, state, and local laws.
