Role Overview As a Hospice RN, you will serve not just as a nurse, but as a clinical leader, advocate, and reliable guide for patients and their families.
You will oversee the care plan, manage symptoms, and ensure that each patient receives tailored, comfort-oriented care that aligns with their goals and preferences. With each visit, you will provide not only clinical skills but also reassurance, clarity, and compassion.
Your Responsibilities
Lead Patient-Centered Hospice Care:
Conduct thorough assessments of patients and their families.
Create and manage personalized, comfort-focused care plans.
Administer skilled nursing care with an emphasis on pain and symptom management.
Serve as the Care Coordinator:
Act as a case manager, coordinating care with the interdisciplinary team.
Collaborate with physicians, social workers, chaplains, and aides.
Monitor patient conditions and adjust care plans to ensure comfort.
Support Patients & Families:
Provide education, emotional support, and guidance throughout the care journey.
Assist families in navigating complex decisions with empathy and clarity.
Establish meaningful, trusting relationships during deeply personal moments.
Drive Quality & Compliance:
Maintain accurate and timely documentation.
Participate in interdisciplinary team meetings and care planning.
Support quality and compliance standards.

What You Bring to the Role
Active RN license (or eligibility for compact licensure).
Graduation from an accredited nursing program.
A minimum of one year of clinical nursing experience.
Strong clinical judgment and a compassionate approach to care.
Ability to independently manage a caseload.
Excellent communication skills, especially in sensitive situations.
Basic proficiency in electronic medical records (EMR).
Current CPR certification.
A valid driver's license and reliable transportation.
