As a Licensed Practical Nurse (LPN), you will have the opportunity to work with a compassionate and dedicated team to provide high-quality care to our residents.
About the Role:
We are seeking a dedicated and compassionate LPN to join our team on a full-time basis, working day shift (7am-7pm). As an LPN, you will be responsible for providing direct patient care, conducting assessments, and communicating effectively with patients, families, and healthcare teams.
Responsibilities and Duties:
Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
Perform admission duties, including completing assessments, MAR, TAR, POS, reviewing chart set-up, and ordering medications
Manage discharges, including completing documentation, notifying physicians and guardians, and arranging transportation
Provide wound care, following protocols, irrigating, packing, and removing dressings
Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
Respond effectively to emergency situations
Requirements:
