The Nurse Navigator serves as a consistent care coordinator, ensuring timely access and seamless coordination of services throughout the continuum of oncology care.

The Navigator will meet with patients and families to provide emotional support and guidance following the diagnosis of cancer through end state disease if necessary.

Provide medical information; Responsible for facilitating: rapid scheduling, coordination follow-up care and tracking patients to ensure completion of recommended treatment plan.

Identify and assist patients and families in eliminating potential barriers to care by identifying and referring to appropriate resources.

Serves as the tumor board coordinator; Active IA RN license

Minimum 1 year oncology experience required

Oncology Nurse Certification preferred

Must be a caring communicator, flexible, able to multitask and be comfortable with technological systems

60 hours per pay period (full-time benefit eligible)

• On-call every 6-8 weeks with nursing rotation
