Montgomery County is seeking a skilled Community Health Nurse II to join the Assessments, Evaluation & Review Services (AERS) team. This role provides clinical assessments and interventions for medically fragile, disabled, and vulnerable individuals—both adults and children—living in the community. The nurse plays a vital role in preventing institutionalization, promoting safety, and supporting independence through timely evaluations and crisis response.

Conduct nursing assessments for State and County programs:

Community First Choice (CFC)

Community Personal Assistance Services (CPAS)

Community Options Waiver (COW)

Medical Adult Day Care Waiver

STEPS (nursing home risk)

PASRR (mental illness/intellectual disability and nursing facility placement)

GES (emergency room psychiatric risk)

Respond to urgent situations involving abuse, neglect, or exploitation of vulnerable adults

Provide crisis intervention, clinical recommendations, and follow-up care planning

The selected candidate will be required to successfully complete a pre-placement background check, medical exam, and drug screening.

Graduation from a National League for Nursing accredited college or university or an equivalent program asdeterminedby the Maryland Board of Examiners with a Bachelor's DegreeinNursing.

Two (2) years of full-time equivalent post-licensure experience as a Registered Nurse, one year of which must have been in the nursing area of Community Health. (Community Health Nursing includes working in schools (School Health Services), Faith-Based Medical Facilities, community clinics, government agencies, and non-profit organizations.).

Possession of a current license to practice as a Registered Nurse in the Maryland State, of Maryland issued by the Maryland Board of Examiners. If the license is temporary, a regular Maryland RN license must be obtained within ninety (90) days of appointment.

The State of Maryland accepts as valid the current RN licensure from a candidate's primary state of residence if that state has an interstate compact with Maryland. Any Registered Nurse (RN) applying for licensure and living in a member state of the Multistate Licensure Compact must take the NCLEX exam in their home state.

The applications of those individuals meeting the minimum qualifications will be reviewed to determine the extent, relevancy of training, and experience in the following areas:

Experience serving at risk seniors and Adults and children with disabilities in a multidisciplinary setting.

Experience working in Nursing Facilities and/ or assisted living facilities.

Experience working in a home care setting and/or providing interventions in the client’s home.

Experience functioning independently in a community setting with clients of all ages.

Experience working with clients with Intellectual Disabilities and/or Mental Illness.

Experience with Community-Based Assessments and Waiver Programs.

Multilingual Proficiency in Amharic, Chinese, French, Korean, Spanish, or Vietnamese.
