Licensed Master Social Worker (LMSW) – Part-Time / PRN

We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our Home Health and Hospice team on a Part‑Time or PRN basis. The LMSW will provide psychosocial assessments, counseling, emotional support, resource coordination, and advocacy services to patients and families experiencing illness, disability, end‑of‑life care, and bereavement. The LMSW will work collaboratively with physicians, nurses, therapists, and other interdisciplinary team members to ensure high‑quality patient‑centered care.

Evaluate social, emotional, financial, environmental, and community support needs.

Develop and implement individualized care plans in collaboration with the interdisciplinary team.

Assist patients and families in accessing community resources, financial assistance programs, and support services.

Educate patients and families regarding available healthcare, hospice, and community services.

Facilitate advance care planning and discussions regarding patient goals and preferences.

Participate in interdisciplinary team meetings and contribute to patient care planning.

Coordinate discharge planning and transitions of care when appropriate.

Ensure compliance with federal, state, agency, Medicare, and hospice regulations.

Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE).

Current Licensed Master Social Worker (LMSW) license in the State of Texas.

Minimum one (1) year of social work experience preferred.

Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work preferred.

Knowledge of Medicare, Medicaid, and community resources preferred.

Compassionate and empathetic approach to patient care.

Proficiency with electronic medical records (EMR) systems.

Knowledge of hospice philosophy and end‑of‑life care principles preferred.

Ability to travel to patient homes, facilities, and community locations.

Ability to utilize a computer, tablet, and other office equipment.

Part-Time and PRN positions available.

Flexible schedule based on patient needs and agency requirements.

Some evening or weekend visits may be required.
