Registered Nurse (RN) - Home Health Care - ASAP

ALL CARE and COMCARE home health provider group has an immediate opening for a PER DIEM (field-based) California-licensed Registered Nurse to work as a home visiting nurse for Fontana, CA and it's neighboring areas. Preferably looking for an IV nurse who is great at peripheral insertions and infusion via PICC/pump, experienced in assessment/charting, a fast learner, and has good communication skills.
This opportunity is also open to a clinically experienced RN who is a self-starter in home health. RN to undertake Admission, assessments, and supervisory visits/ follow-ups.
Will coordinates and directs the home care patient's based on individual patient needs.
Cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician.
Educates patients, families, caregivers and community providers to safely perform care.
Assesses the home environment for safety, infection control, and community resource needs.
Implement a plan of care.
Minimum of one (1) year relevant RN experience.
Recent, relevant experience in a Medicare-certified home health agency as a case manager, or primary nurse is a plus
Familiarity and work experience using home health software documentation application such as MyUnity or is willing to learn.
Registered as a professional nurse to work in California.
Valid California driver's license and current car insurance.
Current IV Therapy skills, with knowledge of venous access devices and ambulatory pumps.
Knowledge regarding the impact of acute and chronic illness on the lives of patients and patient families in the community.
Collaborates with other disciplines to provide care.

Nursing license - California (back to back, or certificate).
