Job Title: Hospice Registered Nurse (RN) - Home Health/Hospice
Position Summary
We are seeking an experienced Hospice Registered Nurse (RN) to provide compassionate, high-quality hospice care to patients in a home-based setting. The ideal candidate will have recent hospice experience and proficiency with Homecare Homebase (HCHB) documentation software.
Schedule
- Monday through Friday
- 8-hour shifts scheduled between 7:00 AM and 6:00 PM
- Participation in on-call rotation for nights and weekends is required
Key Responsibilities
Provide skilled nursing care to hospice patients in accordance with the individualized plan of care
Conduct comprehensive patient assessments and ongoing evaluations
Manage symptoms and provide pain management education and support
Collaborate with physicians, interdisciplinary team members, patients, and families
Educate caregivers regarding disease progression, medications, and end-of-life care
Maintain accurate and timely documentation in Homecare Homebase (HCHB)
Participate in on-call coverage during evenings, nights, weekends, and holidays as assigned
Required Qualifications
Active and unrestricted Registered Nurse (RN) license
Recent hospice nursing experience required
Recent Homecare Homebase (HCHB) experience required
Strong clinical assessment and critical thinking skills
