Full-Time Hospice RN / Registered Nurse
Are you passionate about providing life-changing service to patients? If so, we have an immediate opening for Hospice Registered Nurse.
The Hospice RN plans, organizes, and directs hospice care, with a focus on community health education and experience. As an Hospice RN, you will utilize your nursing skills to provide professional care, assess patient needs, develop care plans, administer medications, and coordinate patient care. You will also have the opportunity to work closely with the interdisciplinary group to ensure the emotional well-being of patients and their families.
Patient Care

Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
Provide professional nursing care using the nursing process
Develop care plans and establish goals based on nursing diagnosis
Assist patients with activities of daily living and self-care
Act as a Case Manager to coordinate patient care
Maintain accurate clinical notes and records
Communicate with physicians and community health professionals
Teach patients and families self-care techniques
Participate in quality assessment performance improvement activities
Graduate of a National League for Nursing accredited school of nursing
Minimum of two years of nursing experience, preferably in public health, home care, or hospice
Current licensure to practice professional nursing
