Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN

Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.

Document all services provided.

Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.

Report changes in patient condition and needs to RN

Current Louisiana Licensed Practical Nurse (LPN) license required

Minimum 1 year clinical experience as an LPN

Reliable transportation to conduct patient visits at their place of residence or facility
