Hospital Liaison, Registered Nurse (RN), Home Health

The Registered Nurse (RN) liaison in Home Infusion plays a vital role in bridging the gap between the hospitals and home infusion services. The position is responsible for fostering strong relationships with healthcare providers and local organizations, ensuring seamless transitions for patients from hospital to home. The home infusion liaison advocates for patients and families, coordinates, educates patients, and promotes home infusion services. The role is key in educating stakeholders about home infusion offerings and ensuring compliance with industry standards, ultimately contributing to the well-being and satisfaction of patients receiving care by home infusion. Baptist Health is looking for a Home Infusion RN Hospital Liaison to join our Home Infusion team at Baptist Health Lexington!
Monday -Friday from 9am -5:30pm.
On call rotating weekends for new referrals leaving the hospital.

Provide guidance on maintaining a sterile environment and proper use of infusion equipment.
Maintain accurate, timely, and detailed documentation of all patient care activities in compliancewith department and regulatory standards.
Healthcare workers must bepresent and punctual for all scheduled shifts to ensure the delivery of high-quality patient care.
Provide education to hospital staff about home infusion services.
Collaborate with the healthcare team, including physicians, pharmacists, and case managers, to develop and implement individualized care plans.
Serve as the primary contact between home infusion and the local health care facility.
Participate in quality improvement initiatives and contribute to the development of best practices for home health care.
Meet established productivity standards by managing caseloads efficiently document appropriately within designated timeframes.
Ensure timely and accurate submission of patient care notes, reports, and other required documentation to meet department and regulatory deadlines.
Balance patient care needs with productivity goals, ensuring high-quality care without compromising efficiency.
Coordinate with the home infusion pharmacy regarding patient care and insurance coverage.
Associate's degree in nursing, BSN preferred.

• Graduate of an accredited nursing program.
• Must have valid KY RN license.
• Work Experience
