UnitedHealth Group, part of the Optum family, seeks a Per Diem LPN/LVN in Home Health to deliver high‑quality patient care in patients' homes. Responsibilities include following the care plan, educating families, and documenting patient status for the care team.

CPR certification is preferred or achievable within 90 days; home care experience and flexible hours are valued. Compensation includes per-visit and hourly pay with a comprehensive benefits package and 401k options.
