Registered Nurse Physicians Immediate Care

Rn - Transplant

Coordinates all aspects of care (inpatient and/or outpatient) - per program assignment, including but not limited to record review, medication refills and/or prior authorizations; patient education, review of test/procedure/lab results. Makes referral for medically necessary services, consults, equipment and supplies. Functions as a liaison between administrative, clinical staff and physicians within the transplant department, as well as other departments involved with the care of patients.

Assists and supports patient discharge planning as needed. Provides patient education regarding care and medications. Provides assistance to patients and/or physicians in clinic and on hospital floors, as needed. Carries out patient care orders as directed by physician and in accordance with hospital policy. Assists with patient admissions and discharges as required.

Requirements: BLS, CA License, Bachelor's, EPIC Phoenix (strict requirement) Outpatient experience in EPIC.

Shift Details: M-F, 5x8s no weekends or holidays Modified Time: 8/24/2026 12:00:00 AM

This position is responsible for coordinating all aspects of the pre and post-transplant and disease management patients including, but not limited to: patient medical management, immediate pre-operative assessment/management, post-operative assessment/management, and discharge planning. Participates in providing educational programs regarding transplantation to hospitals, medical specialty units, patient groups and the community. Experience with transplant populations required.
