Job Duties Provide nursing care to patients in skilled nursing and assisted living facilities

Monitor patient conditions and report changes to RN or provider

Document care accurately and thoroughly in patient records

Collaborate with care teams to deliver quality outcomes and a positive patient experience

Ensure a safe and supportive environment for residents and staff

Requirements Active LPN license in the state of Wisconsin

Minimum of one year of LPN experience in a healthcare facility

Reliable, professional, and able to adapt in different care settings
