Job Summary and Responsibilities
As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.
Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

Accurately collects admission data and records information on Admission/Assessment/History Record within 2 hours of arrival on unit.
Delegates data collection appropriately.
Observes and reports to the Registered Nurse, patient's responses to therapeutic interventions, hospitalization and diagnosis, including physical and psychological response.
Collaborates with the interdisciplinary team in meeting identified needs by appropriate referrals.
Collaborates with the Registered Nurse regarding discharge planning.
Demonstrates individual competency in selected psychomotor skills and independently performs technical procedures such as demonstrated ability to carry out procedures, assist physicians with procedures, safely use equipment, and proficiency on HIS.
Job Requirements
Required

• 1 year SNF/LTC care of the aged, acutely ill,chronically ill and patients in need of restorative care - Wound Care LVN
• Licensed Vocational Nurse: CA
• Basic Life Support - CPR - authorized by the American Heart Association
