Every Monday, Wednesday, and Friday
Position Type: Consistent, part-time home-care assignment

Helping Hearts is seeking a reliable, compassionate, experienced, and highly attentive caregiver to support a client in the Franklin area. This client requires full assistance with activities of daily living, including hands‑on transfer assistance, and is considered a significant fall risk.

We need a caregiver who understands that fall prevention requires constant awareness, close supervision, careful assistance, and proactive safety measures throughout the entire shift.

The selected caregiver must be available to work all three scheduled days Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Consistent attendance is essential because the client depends on a stable routine and familiar caregivers.

The client also has a small dog in the home. At Helping Hearts, caregiving is more than a job, it is a calling. Full Assistance with Activities of Daily Living
The caregiver will provide respectful, hands‑on assistance with the client’s daily personal‑care needs as outlined in the client’s care plan.

Bathing or personal‑hygiene assistance

Grooming and oral‑care support

Toileting and incontinence care

Mobility assistance

Safe transfer assistance

Wheelchair and mobility‑device assistance

General comfort and personal‑care support

Maintaining the client’s privacy and dignity during all care tasks

Care must be provided patiently and professionally without rushing the client. The caregiver must follow the established care plan and use only approved transfer techniques and equipment for which the caregiver has received proper training.

This client requires hands‑on assistance with transfers. Repositioning for safety, hygiene, and comfort

Transfers using approved equipment when required by the care plan

Previous experience using a Hoyer lift, sit‑to‑stand lift, or other mechanical transfer equipment is preferred but not required. Applicants without this experience must be willing and able to complete all required hands‑on training and demonstrate safe use of the equipment before independently performing mechanical‑lift transfers.

During every transfer, the caregiver must:

Move footrests, cords, furniture, and other obstacles out of the way

Use approved transfer equipment when required

Stop immediately if the transfer becomes unsafe

The caregiver must follow the client’s care plan and use any required gait belt, Hoyer lift, sit‑to‑stand lift, or other approved transfer device only after receiving proper training and authorization.

If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction.

Proactive Fall Prevention and Safety Monitoring
Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety‑focused throughout the entire shift.

This is not a passive companion assignment or a position where the caregiver can sit nearby without actively observing and assisting the client.

The caregiver will be expected to:

Remain aware of the client’s location, mobility, and immediate needs

Provide close supervision during standing, walking, toileting, and transfers

Encourage the client to request assistance before attempting to stand or transfer

Check for spills, cords, loose rugs, clutter, poor lighting, and unsafe furniture placement

Never leave the client unattended during a task when supervision is required

Avoid personal‑phone use or other distractions that interfere with active supervision

Immediately report changes in mobility, balance, strength, or transfer ability

Immediately report every fall, near fall, injury, or safety concern

The caregiver must never attempt an unsafe lift or transfer. If the client cannot be moved safely according to the care plan, the caregiver must protect the client from further injury and contact the Helping Hearts office immediately for direction.

Active Engagement and Companionship
The client needs a caregiver who will be present, attentive, conversational, and genuinely engaged. Meaningful companionship is an important part of this assignment.

The caregiver should:

Help maintain a calm, positive, and familiar daily routine

Respect the client’s choices, preferences, personal space, and independence

Provide companionship without becoming distracted by a personal phone, television, or unrelated activities

We are looking for someone who understands that quality caregiving includes both physical assistance and meaningful human connection.

Depending on the client’s care plan and preferences, responsibilities may include:

Encouraging appropriate food and fluid intake

Cleaning food‑preparation areas after use

Observing and reporting appetite changes, swallowing concerns, nausea, or refusal of food or fluids

Following all dietary instructions listed in the client’s care plan

Attention to cleanliness, nutrition, hydration, and the client’s comfort is essential.

Light Housekeeping and Home Support
The caregiver will help maintain a clean, organized, comfortable, and hazard‑free living environment.

Keeping the bathroom and personal‑care areas clean

Returning supplies and mobility equipment to their proper locations

The client’s direct care, supervision, and safety must always remain the caregiver’s first priority.

Observe, Document, and Report Changes
The caregiver must closely observe the client’s overall condition and promptly report concerns to Helping Hearts.

Caregivers must complete accurate shift documentation, electronic visit verification, and required care notes through the company’s approved system.

Concerns must be reported immediately. Caregivers must not wait until the end of the shift to report an urgent change, fall, near fall, injury, or safety concern.

Small Dog in the Home
The client has a small dog. Be comfortable entering and working in a home with a small dog

Remain aware of the dog’s location when the client is walking or transferring

Help prevent the dog, toys, food bowls, or leashes from becoming fall hazards

Follow the client’s instructions regarding appropriate interaction with the dog

Never allow the pet to interfere with the client’s care, supervision, or safety

Routine pet care is not automatically part of the assignment unless it is specifically included in the client’s approved care plan.

Exceptional Reliability and Continuity of Care
This assignment requires a caregiver who can consistently work:

Every Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM.

Frequent call‑offs, late arrivals, early departures, or ongoing schedule changes would disrupt the client’s care, safety, and established routine.

The ideal applicant understands the importance of continuity of care and is committed to being present, prepared, and dependable for every scheduled shift.

Previous Hoyer lift, sit‑to‑stand lift, or mechanical‑transfer‑equipment experience is preferred but not required. Applicants must be willing to complete all required training and demonstrate safe transfer skills before independently using transfer equipment with the client.

Professional and Personal Qualities
The ideal caregiver will be:

Able to follow care‑plan instructions

Comfortable providing hands‑on personal care

Willing to learn and correctly use approved transfer equipment

Able to communicate concerns immediately

Comfortable working around a small dog

Be at least 18 years old

Complete all required orientation and training

Maintain current CPR and First Aid certification or obtain certification as required

Follow all Helping Hearts attendance, safety, documentation, confidentiality, and professional‑conduct policies

Social Security information for payroll and employment‑verification purposes

Applicants may choose which acceptable Form I-9 documents to present in accordance with federal requirements.
