Home Care and Hospice RN/LPN

Responsible for ensuring continuity of care and coordinating discharge planning as a member of the interdisciplinary team by providing direct patient care under multiple levels of care. Collects and documents objective and subjective patient data in accordance with agency policies and procedures and reports data to the directing registered nurse and other members of the health care team so that a plan of care is initiated and changes are accurately addressed.
About the Role
The caregiver will serve patients on the South team, covering Medina and Wayne counties, including Wooster, Medina, Wadsworth, Brunswick, Valley City, Lodi, Orrville and Rittman. Caregivers typically visit and provide care to approximately six patients each day in the comfort of their own homes. The position is scheduled Monday – Friday from 8:00 AM – 4:30 PM, and the caregiver may start and end the day working remotely.
Responsibilities
Ensure continuity of care and coordinate discharge planning.
Act as an interdisciplinary team member by providing direct patient care under multiple levels of care.
Collect and document objective and subjective patient data in accordance with agency policies and procedures.
Report data, as appropriate, to the directing registered nurse and other members of the health care team so that a plan of care is initiated and changes are accurately addressed.
Actively engage with and educate potential customers about the benefits and appropriateness of care.
Ensure the patient and their family have adequate knowledge and an understanding of the hospice or homecare benefit, as appropriate.
Contribute to the assessment of patients by collecting data for analysis by an RN.
Observe patients and report any unusual conditions to professional staff.
Observe patients for safety risks and implement safety measures within scope of practice.
Administer medications, within scope of practice, and observe patients for effectiveness and for adverse reactions to medications or treatments.
Provide direct patient care, including dressing changes and laboratory tests, as part of the plan of care.
Provide ongoing education and information to patients and family members, taking into consideration cultural issues and other special challenges.
Maintain complete and up‑to‑date documentation and files at all times regarding patient condition and the effectiveness and timeliness of care and interventions.
Ensure a seamless transition to the residential setting by coordinating services at time of discharge.
Be flexible in setting and revising priorities for one’s daily assignment based on patient needs/acuity, family requests and available resources.
Minimum Qualifications
Graduate from an accredited practical or vocational nursing program.
Hold a current Ohio license as a Licensed Practical Nurse.
Have one year of clinical experience as a professional nurse in an acute setting.
Hold a Basic Life Support (BLS) certification through the American Heart Association (AHA).
Hold a valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
Have knowledge of JCAHO and Medicare standards and regulations.
Preferred Qualifications
M‑IV certification.
Infusion experience.
Physical Requirements
Requires full range of motion, manual and finger dexterity and eye‑hand coordination.
Requires standing and walking for extensive periods of time.
Requires corrected hearing and vision to normal range.
Requires some exposure to communicable disease or bodily fluids.
Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical demand requirements are in excess of those for light work.
Must be ambulatory and able to drive to patients’ homes, able to climb stairs, pull, push, and facilitate movement of the patient.
Must be able to read, write and communicate with patients.
Personal Protective Equipment
~ Follows standard precautions using personal protective equipment as required.

Pay Range
Minimum hourly: $23.25
Maximum hourly: $33.50
The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking into consideration factors such as work history, experience, skill set and education. The pay range does not include any applicable pay practices such as shift differentials or overtime, nor does it include the value of the Cleveland Clinic benefits package.
