Office Location: 6400 St Johns Ave, Palatka, FL 32177
Schedule: 9a-6p/ 10a -7p/ or 11-8 and every other weekend
Are you a Registered Nurse looking for a new opportunity? Haven Hospice is seeking a passionate, dedicated Hospice Admissions RN to join our team in Palatka, FL. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. Provide 1:1 care to make a lasting impact on patients and families

As a Hospice Admissions RN You will:
Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
Obtain medical history, current medications, and symptoms
Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
Coordinate the start of services (nursing, aides, equipment, medications.)
