Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPNs who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

In collaboration with physician, the NP will be providing comprehensive assessment, including multifactorial complex

As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primarycare services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insuredevelopment of care planning and access to preventive care services in accordance with member and Plan health outcomegoals and objectives. Master's Degree in Nursing; graduate of accredited Nurse Practitioner program

One year of clinical experience in role of nurse practitioner required; Previous clinical RN experience in acute, subacute and/or long term care setting

Prior experience in managed care a plus

NYS RN/NP License, Registration, DEA certificates
