LPN - General Nursing; Full Time

Join YesCare as a Licensed Practical Nurse (LPN) and play a critical role in delivering compassionate and effective patient care within our dynamic healthcare program.

Patient Assessment and Care Planning: Assist in assessing the health status of patients and contribute to the development and modification of individualized care plans.

Direct Patient Care: Provide safe, effective nursing care for multiple patients within the facility’s healthcare program.

Scope of Practice: Perform delegated nursing tasks consistent with educational preparation and within the scope of practice.

Care Coordination and Monitoring: Oversee onsite and offsite patient scheduling in collaboration with the care team, and ensure follow‑up care, including telemedicine appointments and consult processes in accordance with policy and contract requirements.

Quality Improvement & Collaboration: Participate in Quality Improvement initiatives and Patient Care Committee meetings, and coordinate with the Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders to ensure seamless patient care.

Graduate from an accredited Licensed Practical Nursing program.

Current licensure as a Practical Nurse (LPN) in the state of employment.

Familiarity with state laws and regulations governing LPN practice.

Ability to pass a background check, security clearance, and drug screening.
