Schedule
- Shift: 8-hour day shifts only
Assignment Details

• Patient Mix: Adults / Geriatrics
• Visit Expectation: May see up to 6 patients/day; 30 productivity points standard
• Travel: Occasional radius up to ~60 miles

Requirements

• Required: 1 year of skilled home health experience
• Required: Licenses in both OR and WA
• Required Certifications: BCLS / BLS
• Required Clinical Skills: Home Health assessments and OASIS documentation, admissions, lifting and transfers
• Neurologic requirement: Admissions experience required
• Preferred: Wound care, PTA experience

Roles & Responsibilities

• Conduct patient assessments and develop individualized treatment plans.
• Perform home health visits and provide rehabilitation interventions consistent with patient needs.
• Document care and maintain accurate OASIS records as mandated by regulations.
• Communicate effectively with patients and their families regarding treatment progress and needs.
• Coordinate with interdisciplinary team members to deliver comprehensive care.
• Manage ordering of supplies and scheduling of appointments as needed.

Compliance / Notes

• Confirm RTO/scheduling restrictions with the hiring manager.
• Orientation and pre-start modules may be required.
