Villa Healthcare is seeking a Hospital Liaison to join their team! The Hospital Liaison serves as the primary connection between hospitals and the skilled nursing facility (SNF), ensuring smooth transitions for patients requiring post-acute care. This role involves building and maintaining relationships with hospital discharge planners, case managers, physicians, and other referral sources to drive admissions while ensuring that patients receive appropriate care.

Hospital Liaison Duties and Responsibilities:

Evaluates potential admissions by reviewing medical records, communicating with hospital staff, and assessing the patient's needs.

Develops and maintains strong relationships with hospital case managers, discharge planners, and healthcare providers.

Educates referral sources on the facility's services, specialties, and patient outcomes.

May participate in hospital rounds, care conferences, and community outreach events to increase visibility.

Provides regular updates to hospital partners on bed availability, services, and outcomes.

Hospital Liaison Skills and Abilities:

Strong written and verbal communication skills in English.

Broad medical competency and understanding of insurance practices.

Ability to relate positively, favorably, and cooperatively with others, including associates, residents, family members, and personnel of outside agencies and organizations.

Basic-advanced computer knowledge and ability with an aptitude to learn company applications.

Proficiency in CRM software and electronic medical record (EMR) systems.

May require occasional weekend or evening availability.

Travel: Up to 90%.

Hospital Liaison Education and Experience:

A Bachelor's degree in marketing, communication, healthcare administration, or related field strongly preferred.

Extensive healthcare sales work experience may be considered in lieu of Bachelor's degree.

Former sales training preferred.

Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
